Mail Attachments

Struggling a bit with this (probably just a mental deficiency!!).

I need to send an email message that also includes an attachment (pdf file) based upon user specified input to .php page.

How do you specify to add the attachment along with the body text, and related to this, how do you tell php to route this out through a specific smtp server?
